Consent By participating in telehealth services, you consent to:

Limitations Telehealth has limitations:

  • Technology failures may occur
  • Not all conditions can be diagnosed remotely
  • In-person evaluation may be necessary

State Licensing Telehealth services are only provided to patients in states where Dr. Swenson is licensed: California, Wyoming, Florida, Massachusetts, and Vermont.

Payment Terms

  • Fees Fees for services are as posted on our Site or discussed during consultation. Prices are subject to change.
  • Payment Payment is due at the time of service unless other arrangements are made. We accept [payment methods].
  • Subscription Services Subscription services (HRT, Peptide Management) are billed monthly. You may cancel at any time with 30 days’ notice. No refunds for partial months.
  • Membership The Cobalt Circle membership is billed monthly. Membership fees are non-refundable. Membership may be terminated by either party with 30 days’ written notice.

Cancellation Policy

  • Appointments Please provide at least 24 hours’ notice for appointment cancellations. Late cancellations or no-shows may incur a fee.
  • Subscription Services Cancel subscription services with 30 days’ written notice. You remain responsible for the current billing cycle.

Limitation of Liability

To the fullest extent permitted by law:

  • Cobalt Regenerative is not liable for any indirect, incidental, or consequential damages arising from use of this Site or our services
  • Our total liability shall not exceed the amount you paid for services in the 12 months prior to the claim
  • We are not liable for delays or failures due to circumstances beyond our control

Indemnification

You agree to indemnify and hold Cobalt Regenerative, Dr. Swenson, and staff harmless from any claims, damages, or expenses arising from:

  • Your use of the Site
  • Your violation of these Terms
  • Your violation of any third-party rights
